Thank you for enrolling in this Illinois Retired Teachers Association endorsed program. This program is marketed by AMBA. The online enrollment is provided as a convenience for members. Please save this page for your records.
Coverage will take effect the first of the month following the date this enrollment is submitted, provided your initial premium contribution is paid within 31 days after the date you are billed.

If we have any questions regarding your application, we will reach out to you via mail or phone call.
Soon you will receive your welcome kit in the mail from AMBA. This will include any unique plan, group, and certificate ID numbers and information on how to access your account online at www.ambamyaccount.com.
We will begin processing your application. If we have any questions you will receive a call and if we cannot reach you a letter or e-mail will be sent. You will receive a welcome letter with instructions on how to print your ID card or Policy information online.
If you enrolled in a dental plan, Ameritas is your new dental insurance carrier. Please allow up to ten days for initial processing, then you can activate an online account at Ameritas.com. This will allow you to locate providers, view plan details, see claims, as well as view or print an ID Card. For your convenience, ID Cards will also be sent via mail by Ameritas within the next three weeks.
If you enrolled in a vision plan, this is provided through Vision Service Provider . VSP vision plans are paperless. Note that this does not require an ID Card. To use your benefits, simply schedule an appointment with a provider who accepts VSP and identify yourself as a VSP member. Visit https://www.vsp.com/ to locate providers and view plan details.
Once you receive your welcome kit and are able to create an online account, please familiarize yourself with the benefits of your program. Please review potential waiting periods, limitations, and exclusions in detail. If you have any questions, don’t hesitate to call and ask! For dental clients, if you have an upcoming procedure that may cost more than $300 please have your dentist submit a ‘pretreatment estimate’ so you can know exactly what the plan will pay.
